I've just given it a go: unfortunately I could not replicate the error. I've tried to add item 763120 to group 90674 and it worked at the first attempt, I have then removed it to leave the review in the state I've found it. This leaves me basically in the dark, as I can't find out what the error is without causing the error first.
However, I do have a suggestion: from what I've seen, it seems very likely that you haven't searched for new duplicates after importing the last batch of items (the pubmed source), this will explain why you have the need to manually mark so many items as duplicates. Maybe we haven't made this clear enough in the manual: since looking for duplicates is a very resource intensive task, it doesn't happen automatically, but requires the user to manually trigger it when needed.
More specifically, to get the best possible results (and waste the least of time), one should click on "Get new duplicates" one time only, just after importing the last batch of items. It is possible to "get new duplicates" many more times (for example, each time a new source is imported), but this carries the possibility of creating multiple overlapping groups that will make the duplicate handling more complex.
Back on your case: I think your best option would be to click on "Get new duplicates" (I expect this to take at least 5 minutes to complete) and verify what new groups will be created. If all goes well, most of the items you have identified that still need to be marked as duplicates will be now in their own group (this doesn't apply for items that you have already "added manually").
Finally, and for completeness sake, I would like to try to manually add duplicates once more, just to see if the same error happens again, if it does, I would be very grateful if you could send a screenshot to eppisupport@ioe.ac.uk, this might help me to understand what was wrong in the first place.
UPDATE: while I was writing this, you latest message arrived. I see you've done exactly what I was suggesting!
