This map explores primary care interventions presented by a range of health and care outcomes, such as access, experience or health condition. The studies included in this map can be used to design evidence-based and equity-focused primary care interventions, especially those targeting health condition outcomes, for which there is a wealth of evidence. However, from this map we see a need for greater evidence on what works to improve the experience and the health care use (such as secondary health care and community mental health care use) of underserved populations.
